The Camp Lionheart 2026 program application will be available online on SUNDAY, MARCH 1st, 2026. Please check back here for the application link that goes live at 10:00a on Sunday, March 1st, 2026!

See below for a list of things you will need in preparation for the online application (please note that only legal custodial guardians can fill out and sign an application, and that there is a portion of the online application that the camper will need to be present to fill out):

Name, address, date of birth, etc.

Demographic information and family history

Mental health history and trauma history

Grief and death-loss history

Names and contact info of providers (pediatrician, mental health therapist, psychiatrist, etc)

Medications. allergies, and health history

Release forms and informed consent forms to be signed 

Transportation info
